The University of Wisconsin Department of Family Medicine and Community Health provides comprehensive, patient-centered primary care at 18 clinics across south-central Wisconsin. Our clinicians care for individuals and families at every stage of life, offering preventive services, management of chronic conditions, behavioral health support, women’s health, maternity care, and more.

How We’re Organized
Family medicine clinics in our academic health center are supported by a strong partnership between the University of Wisconsin Department of Family Medicine and Community Health and UW Health. Together, these organizations combine academic excellence, community-focused care, and an integrated health system to serve patients across south-central Wisconsin.
University of Wisconsin Department of Family Medicine and Community Health Clinics
Four of our clinics are owned and operated by the University of Wisconsin Department of Family Medicine and Community Health. In addition to providing comprehensive primary care, these locations serve as key training sites for our residency programs and play a central role in shaping future family physicians.
Faculty members at these clinics care for patients and also teach residents and oversee research projects. All residents establish continuity practices in these clinics and care for their own panel of patients throughout their three years of training.
UW Health Family Medicine Clinics
Fourteen of our clinics are owned and operated by UW Health. UW Health clinics provide comprehensive patient care but do not serve as resident teaching sites. Medical students do rotate through these clinics.
Regional Residency Programs
Our family medicine clinical care training extends beyond Madison through regional residency programs in Baraboo and Monroe. These sites provide access to high-quality primary care in rural communities while offering residents and faculty unique opportunities to practice and teach.
