Our clinic is working to help our patients with high blood pressure and diabetes work towards healthier lifestyles through peer support

  • Groups will meet twice monthly group visits from 4:30-6PM co-led by faculty physicians and resident physicians.
    The first visit will be Thursday 1/9/2025. Subsequent visits will be Tuesdays 1/21, 2/4, 2/18, 3/4, 3/18.  
  • The patient pays office visit co-pay – cost of these may be offset some by insurance and grocery incentives.
  • Pharmacist/Nutrition engagement: all participants will be enrolled in our pharmacist driven blood pressure management program to have asynchronous support for BP management, lab ordering and medication adjustments if unable to make every 2 week visits or after the 3 months group visit program.

  • Participants are challenged to exercise at least 1-2 times per week with several options such as:
    • Pool access at GrandStay Hotel & Suites (Mt Horeb, $20/12 sessions)
    • Walk 35,000 steps per week (5,000 steps per day)
    • Theraband Exercises
  • Consider the following apps to help you support you while you strive to meet your fitness and lifestyle goals
  • You can also use a fitness and food log to track your food and beverage intake and log your steps and activity.
