Congratulations to Jeff Huebner, MD (Associate Clinical Professor) on his new role as Chief Medical Officer for the Wisconsin Medicaid program at the Department of Health Services. Medicaid (BadgerCare) currently serves over one million people in the state by providing health insurance and other health programs for low-income residents with growing attention to partnering with health systems and public health to achieve health equity. Dr. Huebner has provided medical director leadership for UW Health’s Medicare ACO and will provide important family medicine perspective and population health experience in his Medicaid work.
Prior to this role, Dr. Huebner served for nearly seven years as the Medical Director in UW Health’s Office of Population Health where he partnered across primary care, the care continuum, and the community to develop and launch care model programs that improve care and lower costs for patient populations at risk for poor health outcomes due to medical complexity and structural barriers to health. Dr. Huebner’s leadership contributed to the launch of home-based primary care, the current clinical pharmacist program, and Connect Rx. His team also contributed to the expansion of Collaborative Care across UW Health’s entire primary care system and recently launched an initiative to expand advance care planning. Dr. Huebner also developed “core standards” for population health programs that now utilize an equity lens for program development and evaluation.
DFMCH looks forward to Dr. Huebner’s continued practice at the Odana Road Clinic while he develops innovative approaches to partnering with clinicians and communities around the state in his important work.
